Declaration: I declare that I wish to take part in the Online Clinic Consultation service operated by Index Medical Ltd through the cholesteroldoctors.co.uk website. Online consultation is provided through online assessment given on our website by GMC registered medical practitioner. The online consultations and treatment suggested are subject to the terms and conditions set out in this agreement.
I give my consent for information about myself, my health and my current medications including, but not limited to, the information I divulge as part of the online consultation assessment to be viewed and exchanged by and between Index Medical employees and a Doctor working on Index Medical's behalf for the purpose of conducting a medical consultation. I understand this information may be exchanged electronically.
You warrant and undertake that all information provided by you in our online consultation assessment given on our website is correct and true.
All medicines prescribed will be based on the consultation assessment completed by you. Therefore, we take no liability for loss or damage arising from our service or medication prescribed if you supply incorrect or incomplete information.
The contents of this web site are directed solely at those who access the site from the UK. Your use of this web site is governed by English Law and subject to the exclusive jurisdiction of the English courts.

Due to the range of third party networks and innovative technology used for this transmission we are unable to guarantee a time of arrival of the consultation information for a Doctor's attention or absolutely guarantee that it will arrive at all.
We are legally only able to dispense medicines to you where we have received a valid prescription issued by a registered UK prescriber. Prescriptions are issued entirely at the discretion of the prescriber. Index Medical Ltd cannot guarantee that a doctor consultation made through this Website will result in a prescription being issued by the prescriber.
We are not responsible for failure of third party companies' inability to deliver medicines to the correct address or within any particular time or date. You are responsible for signing for packages containing medicines but any signature provided by third parties at the delivery address given by yourself will be deemed to be receipt of the medicines and fulfillment of Index Medical Ltd's obligations in dispensing that prescription.
You are responsible for providing valid credit or debit card details which may be charged if you are not exempt from payment. We reserve the right to not deliver medicines to you if your payment details have expired or are invalid in any way. In such cases we will attempt to contact You in such instances to enable You to provide updated information to allow us to dispense the prescription.
If your prescription medicines appear to be damaged or if the delivery appears to be wrong then please contact Rowlands Pharmacy customer care. We will make every effort to replace any damaged products.
